Paid endpoints that return a payment challenge now send it in both the v1 response body and the v2 PAYMENT-REQUIRED header on every refusal, not just some. A client that reads only one of the two was previously told nothing.

We were wrong
A caller whose payment was declined used to be told their free allowance had run out. It now reports the real reason the payment was refused, because that is the only thing the caller can act on.
